The weighted vest will not fix your bones. This will

You walk into the gym and someone is on the treadmill wearing a weighted vest. You have seen it on the promenade, in the park and across half the internet. The idea that has settled in is that the extra weight protects your bones, and that once you are past forty you should own one.

Two reviews published in August answer that with numbers, five days apart. One measured the vest. The other measured strength training. Together they say something awkward for the trend and useful for you.

What the vest does when you already train, measured across nine trials

The journal Osteoporosis International published a meta-analysis on August 14 covering nine randomized trials and 368 middle-aged and older adults. It compared two different scenarios, and the gap between those two scenarios is the whole article.

First scenario: vest plus exercise against carrying on with your normal life. The vest wins clearly there. Bone mineral density improves at the femoral neck and the lumbar spine, and body fat drops by 2.67 kg on average.

Second scenario, the one that applies if you already train: adding the vest to an exercise program you were doing anyway. There it delivers 0.73 kg more fat lost and no confirmed bone benefit. That bone finding also comes from a single study, something the authors point out themselves.

Bone does not answer to the weight you carry, it answers to the load it has to stop

Living bone behaves like a material that reinforces itself exactly where it gets pressed. It needs a mechanical signal that goes beyond what it already handles every day. If your skeleton has spent decades carrying your body weight as you walk, walking with slightly more is not a new signal. It is the same song played a little louder.

A squat, a deadlift or a leg press change that signal instantly, because the force crossing your hip and your spine multiplies within a few seconds. That is a message bone cannot ignore. The vest does the opposite: it spreads a small load across your shoulders and holds it there for an hour.

Three days at moderate intensity, the bet that wins almost everywhere

The journal Calcified Tissue International published a network of 34 randomized trials on August 10, covering 1,746 middle-aged and older adults. Instead of comparing two options, it compared all of them against each other to rank which mix of intensity and frequency works best at each bone.

The option that ranks first at most sites is strength training at moderate intensity three times a week. Against doing nothing, it significantly improves the femoral neck and the trochanter, two parts of the hip. The other winner is high intensity twice a week, which ranks first for total body density.

Why the answer changes depending on which bone we mean

Bone density is not one single number. It gets measured at specific sites, and each site takes a different load depending on what you do. That is why one routine can move your hip and leave your spine untouched, and why studies look like they contradict each other when they are really talking about different bones.

It is also why a bone plan cannot be one machine repeated. If you want to reach hip and spine you need lifts that load both, with your trunk holding the weight rather than seated and propped against a backrest.

What these studies do not prove

The authors of the 34-trial network flag something worth reading in full. At the lumbar spine, the total hip and Ward's triangle the options did not differ significantly from each other. The order that comes out is a probability ranking, not a race with a clear winner, and they ask for it to be read with caution.

In the vest meta-analysis, the only conclusion rated high certainty is the one about body fat. The bone results sit at low to moderate certainty. Both reviews were run in middle-aged and older adults, so if you are thirty-five you are at the edge of that evidence, not in the middle of it.

So do I throw the vest away?

No. It does what it does. If you already train and you add it to your walks, the evidence says you will lose a bit more fat, and that is the most solid number in the whole meta-analysis. It also turns an easy walk into something that costs you, which for plenty of people is the difference between moving and not moving.

What it is not, is a plan for bone. Buying it as a replacement for the barbell means swapping something that works for something that appeals more. If you wear it, wear it on top of your training and never instead of it.

What moderate intensity means when you are the one lifting

Moderate does not mean comfortable. In practice it is a weight you finish the set with clean technique, while knowing you had only a few reps left in the tank. If you finish fresh and could repeat the whole set without resting, you went too light. If the last rep twists and you rescue it with your back, you went too heavy.

And here is the part almost nobody writes down: that weight stops being moderate the moment you get stronger. What was hard in March is your warm-up today. If the number on the bar has not moved since then, you are no longer training at moderate intensity, you are repeating a movement your body already knows.

One warning that earns its place. If you have been diagnosed with osteoporosis or you have had a fracture, none of this replaces what your doctor tells you. In that case loads and lifts get chosen on clinical grounds, not from a ranking of studies.

What this looks like in your week

Three strength sessions a week, with a weight you can move with clean technique but that makes the last set hard. Lifts where the load travels through your hip and your spine. And a written progression, because bone responds to the weight going up over time, not to repeating the same routine for two years.
