You move all day at work. Your brain gets nothing out of it
If your job keeps you on your feet for eight hours, hauling things, climbing stairs or covering ground, you have probably figured that counts as exercise. You move, you sweat, you finish the day wrecked. It sounds like the body is getting what it needs.
In September, The Lancet Public Health ran a meta-analysis that splits movement by where it comes from, and the answer stings. Movement you choose in your own time lines up with less dementia. Movement you do at work, in the same data, points the other way. This is not a niche question either: Women's Health alone has published fifteen pieces on the brain and dementia in the past three months, and mindbodygreen has spent a while asking whether walking is enough to keep your head sharp.
Four million people, ten years of follow-up, and one result that splits in four
The review pools 74 studies and 4,227,297 people, median age 67, followed for a median of ten years. Its funders include the National Institutes of Health in the United States.
The headline is the one everybody expects. The people who move most carry a 20% lower risk of dementia, 21% lower for Alzheimer's and 29% lower for vascular dementia. No surprise there.
The surprise lands when the authors ask where that movement came from. Leisure-time activity, the kind you pick yourself, tracks with a 24% lower risk. Housework tracks with 15% lower, though that comes from a single study. Work activity tracks with a 20% higher risk. And commuting, 10% higher.
Why movement at work fares worse than movement in the park
Say the obvious part first: this does not prove that a physical job damages your brain. These are studies that follow people for years and watch what happens, without deciding who moves and who does not. They measure that two things travel together, not that one causes the other.
That said, the difference between the two kinds of movement is easy to see. You do not choose the one at work: not the intensity, not the length, not when to stop. It is repetitive, it runs all day, and it rarely lifts your heart rate enough for the heart to adapt. It also arrives with everything else a physical job brings, which is shift work, fewer hours of sleep and less room to recover. The one in the park works the opposite way. It has a start and a finish, you set it, and rest usually sits around it.
That asymmetry already has a name in heart research: the physical activity paradox. This meta-analysis suggests something similar happens in the brain.
Strength gets in through a different door
Ageing Research Reviews published a separate review that looks only at strength training in healthy adults, and it is the other half of this story. It does not measure dementia risk over ten years. It measures how your head works right now.
It finds that strength work improves global cognition and three specific things. Executive function, which is planning and not chasing the first distraction. Working memory, which is holding a number in your head while you do something else. And processing speed, which is how fast you react to something new.
It finds something just as useful in what it rules out. Episodic memory, the kind that recalls where you left your keys or what you ate on Tuesday, does not improve consistently. Strength training does not fix everything, it fixes a specific slice. The authors themselves ask for caution because the studies vary a lot.
What these studies do not prove
The authors of the Lancet Public Health review rate the certainty of their own evidence as very low to low across every domain. That does not mean the work is sloppy. It means the underlying studies do not agree closely enough to state anything flatly.
There is more fine print. The work figure comes from five studies and the commuting one from two. Housework comes from a single study. Set against twenty for leisure time, those are thin foundations. And 93% of the studies were run in high-income countries, so they describe one particular kind of job and one particular kind of free time.
Median age of participants was 67. At forty-five you still have plenty of runway, but you are not the profile that was measured here either. What works in your favor is exactly what these studies track, which is the ten years between what you do today and what shows up later.
What counts as leisure exercise and what does not
The difference is not the clothes or the place. It is three things. That you decide it. That it carries an intensity you notice, not the one the workday hands you. And that it ends, so the body gets time to answer.
That rules out counting the steps you take at work as your training. It also rules out the idea that you already move enough. You may move a great deal and none of that movement may be the kind that comes out well in this data.
What this looks like in your week
Two or three strength sessions, chosen by you, with a load heavy enough to make you pay attention to what you are doing. That is where the effect on attention and working memory described by the Ageing Research Reviews paper lives.
Plus some continuous movement that genuinely lifts your heart rate, not the walk you take while talking on the phone. Bike, treadmill or a hill, it makes no difference. What counts is that it is a slot you protect, not a slot left over.
What does not add up is the workday itself. If you arrive off eight hours on your feet, the body is tired and the brain still has not received what this data rewards. It is unfair and it is how it is.

Part of this does not get solved by knowing more. Protecting two or three slots a week so work cannot eat them is a scheduling problem, not an information problem, and it falls into place faster when someone helps you build it.
