Strength and cardio in the same week: the structure that works best according to evidence
The most expensive belief in the gym: if it doesn't hurt, it doesn't work
If you've ever left the gym thinking "I haven't destroyed myself enough," you're probably wrong. And I'm not saying it, more than 30,000 people who've participated in strength training studies over the last few decades are.
In March 2026, the American College of Sports Medicine published its first strength training update in 17 years. They reviewed 137 systematic reviews, crossed data from tens of thousands of participants, and reached a conclusion that's going to make a lot of people uncomfortable: most people train harder than necessary and more days than they need to.
That doesn't mean training hard is bad. It means there's an enormous difference between training with intelligent intensity and destroying yourself in each session believing that's how you gain more. That difference is what separates someone who progresses month after month from someone who stagnates, gets injured, or simply stops going to the gym because their body can't take it.
This article translates the key findings from that review, and from another recent study with 67 studies and 2,058 participants, into concrete decisions you can apply in your next session. No unnecessary jargon, no hype, and without telling you that you need a 47 page plan to start seeing results.
137 reviews and more than 30,000 people: what the ACSM just published
To understand the weight of what we're talking about, it's worth knowing what exactly this document is. The American College of Sports Medicine is the most recognized scientific authority in exercise science globally. Its previous stance on strength training was from 2009. Seventeen years without updating its recommendations.
The new review, published in Medicine and Science in Sports and Exercise in April 2026, analyzed 137 systematic reviews. These aren't 137 individual studies: they're 137 reviews, each of which already groups multiple studies. The result is a synthesis of evidence covering more than 30,000 participants and addressing practically every question someone can ask about strength training: how many sets, how many days, how much load, whether to reach failure, whether machines work, whether age matters.
The general conclusion is more liberating than restrictive. It doesn't tell you that you need to do more. It tells you that many things you believed were mandatory aren't, and that what really matters is simpler than the fitness industry has sold you.
Training to failure is not mandatory, and it probably slows you down
Let's start with the most rooted myth: that every set has to end when you can't anymore. Reaching muscle failure means doing reps until you physically can't complete one more. For years it's been sold as the only serious way to train. If you didn't reach failure, "you weren't training for real."
The ACSM data tells another story. After analyzing the available evidence, researchers found that muscle hypertrophy isn't affected by training to or not to absolute failure. Strength gains don't change significantly either. What does change is what happens to your body between sessions: accumulated fatigue is much greater when you train to failure systematically.
Think of it this way: if in each set you empty yourself completely, the quality of the following sets drops. If the quality of your third and fourth set is poor because you're already destroyed from the first ones, the effective volume of your session decreases even though on paper you did "all the sets." Training to failure isn't free. It has a cost that many times exceeds the benefit.
This doesn't mean you should never approach failure. It means you don't need to live there. The recommendation based on evidence is to work with what's called "controlled proximity to failure": stop when you have one, two, or three reps left in the tank. Not before, because you wouldn't have given enough stimulus, and not after, because you'd be paying an unnecessary price in fatigue.
2-3 RIR: the zone where your muscle grows without your body paying extra
RIR stands for Repetitions In Reserve: the reps you have left before reaching failure. If you can do 10 reps with a weight and stop at 8, you have 2 RIR. If you stop at 7, you have 3 RIR.
The ACSM recommends working with a target of 2 to 3 RIR as a general reference. That means each set should end feeling hard but not impossible. You should be able to do two or three more reps if forced, but you choose to stop there deliberately.
How do you know how many reps you have left? At first, you don't. And that's okay. Your ability to estimate your RIR improves with practice. The first few weeks you'll probably get it wrong: you'll think you have 3 left and actually have 5, or vice versa. Over time, you learn to read your body's signals and the estimate becomes more precise.
A practical trick: if the last rep of your set moves at the same speed as the second to last, you probably have more than 3 in the tank. If the last one moves noticeably slower, you're in the 1 to 2 RIR zone. If the bar stops or almost stops, you're at or very near failure. The goal is to be in that middle zone where you feel real effort but the movement still has quality.
This has a direct practical consequence: you can train more frequently without destroying yourself, because each session leaves less residual fatigue. And as we'll see later, consistency over weeks and months matters much more than the brutality of a single session.
How many sets per week do you need? What the evidence says about volume
Training volume, how many sets you do per muscle group per week, is one of the variables that generates the most debate. Are 6 sets enough? Do you need 20? Is more always better?
The ACSM establishes a minimum of approximately 10 sets per muscle group per week to optimize hypertrophy. Below 10, results are smaller. Above 18 to 20 sets weekly, researchers observed a plateau effect: you keep working harder, but the additional gain gets smaller and smaller.
A second study published in Sports Medicine in February 2026 by Pelland and collaborators confirms this with an analysis of 67 studies and 2,058 participants. They found that the probability that volume increases hypertrophy is 100%, but with diminishing returns. That is: the first sets of the week give you a lot. The last ones give you less and less. And at some point, adding more sets adds nothing and can subtract if fatigue accumulates.
What does this mean in practice? If you train a muscle group with 10 to 15 sets weekly spread across 2 or 3 sessions, you're in a very good zone. You don't need to do 25 sets of chest per week to grow. In fact, it's likely that with 25 sets you're generating more fatigue than your body can process, and that slows your progress instead of speeding it up.
For someone who trains 3 days a week doing full body, that might mean 3 to 5 sets per muscle group in each session. For someone doing a 4 day split routine, it might mean 5 to 8 sets per muscle in each session of that muscle. The math is simple: what matters is the weekly total, not how much you do in a single day.
2, 3, 4, or 5 days: frequency matters less than you think
This is where many people resist. If you come from the culture of "you have to go to the gym 5 or 6 days to grow," hearing that 2 or 3 might be enough sounds like an excuse. But science doesn't work with excuses.
The ACSM data is clear: when the total weekly volume is the same, the difference between training a muscle group 1 time a week or more than 5 times a week doesn't produce significant differences in hypertrophy. What matters most isn't how many days you go, but how much total work you do for each muscle throughout the week.
Pelland and collaborators' study adds a bit of nuance: it found that frequency can indeed have a positive effect on strength, not on muscle size. That is, if your goal is to be stronger, not necessarily bigger, spreading your work across more days can help. But for pure hypertrophy, frequency is secondary to volume.
What does this mean for your real life? If you have time to go to the gym 4 or 5 days, great: you can spread your volume comfortably and each session will be shorter. If your life only allows 2 or 3 days, that's fine: concentrate your volume in those sessions and the results will be comparable. The important thing is that the total volume is there.
What doesn't work is going to the gym 5 days doing weak 20 minute sessions believing that "the number of days" saves you. It also doesn't work going 2 days doing sessions so long that fatigue stops you from performing. The balance between frequency, volume per session, and recovery is what separates a plan that works from one that only looks like it works.
The load doesn't matter as much as you think, if the effort is there
One of the biggest surprises of the ACSM review has to do with the weight you use. For decades, the classic prescription has been "between 65% and 85% of your maximum rep for hypertrophy." Moderate to heavy weights, always.
The data says something different: hypertrophy isn't significantly affected by the load, as long as the effort is sufficient. Studies with loads as low as 30% of the maximum have produced muscle gains similar to studies with 80% loads, when both groups trained with effort close to failure.
This doesn't mean the weight is irrelevant. If your goal is maximum strength, the ACSM does recommend high loads, at least 80% of your maximum. And for power, the optimal range is between 30% and 70%. But if what you want is to gain muscle size, you have much more flexibility than you thought.
When is it useful to know this? When you're recovering from an injury and can't use heavy weight. When you only have access to light dumbbells, on a trip, at home, in a hotel. When you've been using heavy loads for a while and your body needs a break from the joints. In all those cases, you can keep stimulating your muscle to grow using lighter loads, as long as the effort intensity is high.
The key is proximity to failure, not the number on the plate. If you do 25 reps with a light weight and stop when you have 2 or 3 more in you, your muscle gets a growth stimulus comparable to doing 8 reps with heavy weight and stopping with 2 or 3 in reserve. The mechanism is different, but the measurable result is similar.
Machines, bands, body weight: everything counts if the intensity is correct
Another belief this review puts in its place: that only bars and dumbbells "for real" produce results. The reality is that strength wasn't affected when comparing machines to free weights in the analyzed studies. Elastic bands improved strength. Home workouts improved strength. Circuits improved strength.
The mechanical stimulus your muscle needs to grow doesn't care about brands or equipment. It cares about tension, effort, and progression. If a gym machine lets you work a muscle with sufficient intensity and progress week after week, that machine is as valid as an Olympic bar.
This has important implications for different profiles of people. If you're starting out, machines are an excellent tool because they guide the movement and reduce injury risk while you learn. You don't need to feel intimidated by the free weight area to start progressing. If you've been training for a while and need variety, incorporating bands or body weight work isn't "stepping down." It's giving your body a different stimulus that can be just as effective.
What does matter is that the tool you use lets you apply sufficient resistance and progress over time. A band too soft that never challenges you won't do anything. A machine where you put the same weight for months won't either. The tool changes, but the principle doesn't: sufficient effort, real progression, and your muscle responds.
From doing nothing to doing something: the jump that changes your body the most
If there's a message the researchers repeat with more emphasis than any other, it's this: the biggest gain isn't between "training a lot" and "training way more." It's between "doing nothing" and "doing something."
The numbers back it up. The ACSM review found 26 reviews including 23,204 participants confirming that standard strength training improves strength compared to not training, with evidence quality of 73%. For hypertrophy, 12 reviews with 14,924 participants showed improvements with evidence quality of 79%.
This means that if you don't train strength today and start tomorrow with two sessions a week doing the basics, you're going to experience real changes in a relatively short time. More muscle, more strength, better bone density, better metabolic health. All with the minimum investment of two weekly sessions.
If you already train regularly, the margin for improvement is more gradual and depends on adjusting variables: volume, load progression, rest, food. But if you haven't started yet, don't wait for the perfect plan. Any reasonable plan that gets you started is infinitely better than the perfect plan you never execute. The data is unequivocal on this.
Rest between sets: the myth of 60 seconds
Another surprise from the ACSM review: rest between sets doesn't seem to significantly affect either strength or hypertrophy. Studies comparing short rest, less than a minute, with long rest, more than a minute, found no clear differences in results.
This goes against a very popular fitness recommendation: "rest 60 seconds for hypertrophy, 3 minutes for strength." The reality, according to available data, is that your body doesn't respond so mechanically to the stopwatch seconds as we've been told.
Does this mean rest doesn't matter? Not exactly. What matters is that you rest enough to perform in the next set. If you rest only 30 seconds and in the next set your performance drops 40%, the effective volume of that set is much lower. If you rest 3 minutes and maintain performance, you accumulate more quality work.
The practical recommendation is simple: rest what you need to be able to do the next set with quality. For big compound exercises like squats or deadlifts, that's usually 2 to 3 minutes. For smaller isolation exercises like a bicep curl, it might be 1 to 2 minutes. But don't get obsessed with the stopwatch. Your performance in the next set is a better indicator than the seconds on the clock.
Full range of motion does matter, and here's the data
Within all the variables the ACSM analyzed, there's one that does show a clear difference: range of motion. Strength is positively affected when you train with full range of motion compared to partial range.
This means half squats aren't the same as full squats, at least in terms of strength gains. And it makes sense: when you use the full range, your muscle works in more positions, generates more total tension, and adapts more completely.
In practice, this translates to something very concrete: lower to where your mobility allows with good technique. Don't do half reps with more weight believing that's how you "train stronger." You're training less range and losing stimulus.
There are legitimate exceptions. If an injury limits your range, work in the range you can without pain. If you're doing a specific isolation exercise in a partial range for programming reasons, there's logic to it. But as a general rule, full range produces better results than partial range. The data is consistent on this.
As you get older, this matters more, not less
The ACSM confirms that strength training is safe for healthy adults of all ages. The reviews included participants under and over 55 years old, and the recommendations apply to both groups.
But there's a nuance that's often missed: the older you get, the more important strength training becomes. Age-related muscle loss, sarcopenia, starts earlier than most people think and accelerates after 40. Every decade without strength training is a decade in which your body loses muscle, bone density, and functional capacity.
The good news is that data shows the strength training response stays active throughout life. A 60 year old who starts strength training experiences real improvements in strength, muscle mass, and quality of life. Not at the same rate as a 25 year old, but significant and measurable improvements.
If you're over 40 and still don't train strength, the evidence suggests this might be the single most important health habit you can add. You don't need to squat with 100 kilos. Just start: machines, bands, body weight, whatever you have available, 2 or 3 times a week, with gradual progression. Your 60 year old body will thank you.
Three mistakes that slow your progress without you realizing it
After reviewing all this evidence, there are three patterns that appear again and again in people who train but don't advance.
The first: confusing being tired with having trained well. If you leave the gym destroyed but can't train at the same level the following week, your fatigue is stealing your progress. The goal isn't to leave exhausted. It's to leave having given enough stimulus for your muscle to respond. Remember: 2 to 3 RIR, not zero.
The second: using the same weight for months. Your muscle adapts. If you don't give it a reason to keep growing, more weight, more reps, or more sets, it stays where it is. Progression doesn't have to be dramatic. Half a kilo more on the bar, one more rep with the same weight, half a set more per week. Small sustained increases over time are the most reliable way to progress. The ACSM review uses the word "progressive" as one of the pillars of any effective program.
The third: ignoring recovery. Little sleep, poor food, or not resting between sessions isn't discipline. It's sabotage. Your muscle doesn't grow while you train. It grows while you rest. If you don't give it time and resources to repair and adapt, the stimulus you gave it in the gym is wasted. No strength training program works well on top of 5 hours of sleep and poor food.
The simplest plan that can work, according to what the data says
If you had to reduce everything that more than 30,000 studied people and 137 scientific reviews say to a plan you can apply next week, it would be something like this.
Train strength 2 to 3 times a week. If you can go more days, spread the volume so each session is more manageable. If you can only go 2 days, do full body sessions and make sure you cover the main muscle groups. Aim for a minimum of 10 sets per muscle group per week spread across your sessions. You don't need more than 15 to 18 for most cases.
In each set, use the weight or tool you have available. Bar, dumbbells, machines, bands, body weight: everything works if the intensity is correct. Work with full range of motion. Stop each set when you have 2 to 3 reps left before failure. Not before, insufficient stimulus, and not much after, too much fatigue with no additional benefit.
Rest between sets what you need to perform in the next one. For big exercises, 2 to 3 minutes. For small exercises, 1 to 2 minutes. Don't be enslaved to the stopwatch.
When you can complete all planned reps easily, when your RIR starts to go above 3 without you changing anything, increase the weight a bit, add a rep, or add half a set. Progress gradually. The body responds to progression, not brutality.
Outside the gym: sleep enough, eat enough protein, at least 1.6 grams per kilogram of body weight per day is a solid reference, and maintain reasonable food habits. You don't need a perfect diet. You need a diet that doesn't sabotage your training.
Your muscle doesn't need you to punish it. It needs you to train it with your head
If you've read this far, the central idea should be clear: the largest and most up to date evidence that exists on strength training says you don't need to destroy yourself to grow. You don't need to reach failure in every set. You don't need to live in the gym. You don't need expensive equipment or complicated programs.
What you need is consistency, sufficient effort, not maximum, gradual progression, and patience. Results come if the stimulus is there and the body has time to respond. That's not an opinion: it's what 137 systematic reviews analyzed by the world's largest authority in exercise science say.
